The CAT 3126 was Caterpillar’s first medium-duty diesel engine to benefit from 100-percent electronic control. Debuting in the late 1990s, it succeeded the Caterpillar 3116 engine. Most mechanics and owner-operators appreciate the fact that the 3126 employs CAT’s HEUI injection system (hydraulic electronic unit injector) rather than the camshaft-driven (and often finicky) mechanical rack system used on the 3116 (MUI). However, while the 3126 is generally renowned for its reliability, HEUI pumps and an infamous high-pressure oil line failure are relatively common, as are leaky cam followers covers and overheat scenarios in older (or neglected) engines. And who could forget the potentially catastrophic oil pump idler gear issue or the occasional injector failure? We’ll cover all of that and more below.

Problem #1: HEUI Pump Failure

Caterpillar 3126 HEUI Pump Hydraulic Diesel Fuel Injection System
Also known as the HPOP in other engine applications
(for high-pressure oil pump), the 3126 engine’s HEUI pump is the heart of the HEUI injection system. It’s tasked with supplying adequate oil volume to the fuel injectors. Within the HEUI system, the fuel injectors require highly pressurized engine oil to actuate the fuel side to carry out the engine’s power stroke. Unfortunately, HEUI pump issues are common and can sideline the truck, boat, or piece of equipment the 3126 is powering. On top of that, outside of a pump guru’s hands they aren’t thought to be serviceable or repairable. Last but not least, they aren’t cheap to replace.
What Happens

Heui Pump Failure Cat 3126 Diesel Engine Injection System
Hard starting, rough running, intermittent stalling, and a code 39 all point toward a HEUI pump checking out on the CAT 3126. Ingestion of foreign debris, lack of oil maintenance and high operating hours are the common causes of HEUI pump failure. As for the pump itself, three different styles were offered on the 3126. On most engines, a HEUI pump with a round accumulator (i.e. round top) will be found. However, on later model engines a square top style pump is employed. One key difference is that the round top HEUI pumps have a removable (and thereby replaceable) injection actuation pressure control valve (IAPCV), the pump’s external driver (more on the IAPCV in a bit).

Luckily, and depending on how destructive the HEUI pump failure is, a bad HEUI pump doesn’t always damage the fuel injectors downwind of it. However, that doesn’t make pump replacement any less costly. New HEUI pumps range from roughly $1,100 in the aftermarket to as much as $2,700 from a CAT dealer. Higher volume, heavy duty HEUI pumps are also available in the aftermarket, and the one pictured comes from DieselSite, a company with an extensive background in cracking open HEUI pumps and redesigning and/or upgrading internals for improved longevity and increased displacement. With any HEUI pump replacement, always buy this vital component from a reputable source.
Problem #2: High-Pressure Oil Supply Line

ScreenshotCat 3126 High Pressure Oil Supply Line Failure Heui Injection System
One of the CAT 3126 engine’s most notorious failure points is its high-pressure oil supply line. This hose routes highly pressurized oil (as high as 3,500 psi) from the HEUI pump outlet to the oil rail that feeds the fuel injectors (the rail is located within the cylinder head). If you recall, adequate high-pressure oil delivery is crucial for the HEUI fuel injectors to perform at their peak. In addition to any leak or loss of desired pressure from this location leading to a mess of engine oil under the hood, due to how the HEUI injection system is designed, a substantial oil leak will eventually shut the engine down.
What Happens

3126 Cat Diesel Engine Heui High Pressure Oil Line Failure
After the earliest versions of the CAT 3126 left the assembly line with a steel high-pressure oil supply line, Caterpillar switched to a steel braided rubber hose. Engines equipped with the latter change are the versions most prone to suffering from leaks. Over time, the steel braided rubber line becomes brittle and deteriorates. Oftentimes it begins to leak but ultimately worsens and sprays highly pressurized engine oil all over the engine compartment. On a hot engine (especially with the kind of heat the turbo and exhaust manifold see) this poses a potential fire hazard, in addition to a huge mess.
Solution(s)

Heui Injection System Steel Line Update Cat 3126 Diesel Engine
Realizing the problem, Caterpillar addressed it by releasing a high-pressure oil supply line update kit. The kit includes a steel line to replace the problematic rubber one, and also features quick connect fittings. Beyond that, CAT reverted back to the original style, steel high-pressure oil line being installed on the 3126 on the assembly line. Based on the potential fire hazard alone, if you own an early CAT 3126 with the rubber high-pressure oil line it behooves you to install the steel line update kit. In fact, it’s the first item to inspect, identify, and (if need be) address on any 3126.
Problem #3: Leaking Cam Follower Covers

Cat 3126 Diesel Engine Leaking Camshaft Follower Covers
Oil leaks are an eventuality for virtually every diesel engine. On the CAT 3126, oil leaks are most prevalent where the cam follower covers bolt to the block. It’s such a well-documented leak point on the 3126 that most secondhand engine buyers replace these gaskets before addressing anything else. Part of the reason for this is that the cam follower cover gaskets are time consuming to access and replace while the engine is installed in a truck, boat, or piece of heavy equipment. A unique design, the cam followers are attached to the cam follower covers, so they should always be handled with care.
What Happens

Cam Cover Plate Caterpillar 3126 Diesel Engine Parts
After thousands of heat cycles, operating hours, miles, and years, the gaskets that seal the cam follower covers to the engine become brittle, shrink, and begin allowing oil to escape. It’s easily noticeable during any routine, external engine inspection, and will progressively become worse with time. There are three cam follower covers on the CAT 3126, each with its own separate gasket. Unfortunately, replacing them calls for the removal of the air compressor, power steering pump, HEUI pump, and ECU, followed by the rocker assemblies and pushrods being pulled out in order to access the cam follower cover bolts.
Solution(s)

Cat 3126 Diesel Engine Camshaft Follower Cover Gasket
Due to essentially having to strip the intake (driver) side of the engine down in order to reach them, a cam follower cover gasket job is highly involved as far as labor is concerned. Once removed, each cover’s mating surface has to be carefully cleaned, as does the mating surface on the block, prior to installing the new gasket (PN 4Y-9652). In addition to the new gaskets, a thin layer of high temperature gasket maker ensures a trouble-free seal for years to come. It’s also worth noting that, once the covers are bolted back in place and torqued to spec (20 ft-lbs), the rocker assemblies and pushrods have to be installed, followed by setting the valve lash.
Problem #4: Overheating

Coolant Temperature Gauge Cat 3126 Diesel Engine Overheat
Overheating can have some of the most devastating trickle-down effects in a diesel engine. Not only can boiling coolant sideline your truck, boat, or equipment, but it can also warp the cylinder head enough to blow the head gasket, crack the head or, worse yet, lead to an in-cylinder issue. With the CAT 3126, most overheat situations occur in motorhome applications. In these diesel pushers, a plethora of overheating problems can plague the engine. From airflow restrictions to improper diagnosis to incorrectly installed components, the overheat issues encountered here tend to run the gamut.
What Happens

Motorhome Cat 3126 Diesel Engine Compartment Pusher RV
Excessive coolant temps can occur for various reasons, but here are some of the most common in CAT 3126 powered RV’s. 1) An engine cooling fan that’s been installed backward (with airflow blowing in the wrong direction), 2) an engine cooling fan that fails to engage, 3) a plugged cooling stack (with debris blocking airflow through the radiator, intercooler, and/or condenser), 4) a defective or damaged water pump (think chipped impeller fins, an impeller spinning on the shaft, or a high-mile unit checking out on account of old age), 5) sticking thermostats (there are two on the 3126, unlike the 3116’s use of a single unit). Other causes stem from low coolant, the wrong coolant type, or a bad coolant temp sensor.
Solution(s)

Engine Cooling Fan Failure Caterpillar 3126 Diesel Shroud
All of the above can make troubleshooting a 3126 overheat condition less than fun, but a trained CAT mechanic or someone familiar with these engines shouldn’t be stumped for long. As with many common failure points on an engine, steps that help avoid the issue to begin with are always the best advice. Things like ensuring the correct coolant level and coolant type are in play here, as is addressing any coolant leaks promptly upon discovery, and making certain the engine fan is both operating as commanded and moving air in the correct direction. Last but not least, remember that any time the thermostats see excessive coolant temp they can become damaged and never work correctly again.
Problem #5: Failed Oil Pump Idler Gear

Cat 3126 Diesel Engine Oil Pump Idler Gear Failure
Few issues can damage an engine quicker than a lack of oil pressure. Without adequate pressure, the crankshaft’s main bearings and the connecting rod bearings become starved and begin to wear. Many 3126 engines are notorious for producing what can only be called “low oil pressure.” At both idle and operating rpm, a lot of owners report seeing oil pressure that checks in slightly lower than CAT’s listed specifications. This is common for early versions of the 3126 oil pump, but can also be a sign of oil pump problems.
What Went Wrong

GMC Top Kick Medium Duty Truck Cat 3126 Diesel Engine Low Oil Pressure
Aside from early 3126 oil pumps inherently producing low (ish) oil pressure from the get-go, it’s highly common for the idler gear to cause excessive wear inside the oil pump. This friction can result in metal debris becoming present and circulating throughout the engine oil circuit. Of course, the drop in oil pressure means less oil flow feeding the main and rod bearings, which in turn can begin to wear. Unfortunately, many mechanics discover excessively worn rod bearings (and sometimes even main bearings) after dropping the oil pan.
Solution(s)

Cat 3126 Diesel Engine New Oil Pump Low Pressure Replacement Parts
The best solution for this problem (provided you catch it and address it in time) is to replace the engine’s original oil pump with CAT’s updated version. The updated oil pump, which can be found in C7’s, features a different internal design and delivers higher oil pressure throughout the rpm range. The new pump isn’t cheap by any stretch of the imagination (upward of $1,800 at many CAT dealerships)(PN 496-4800), but it’s an upgrade that’s 100-percent worth the peace of mind it offers. On the proactive side of things, it always pays to inspect the oil pump idler gear for excessive play any time the oil pan is off a 3126.
Problem #6: Failed Camshaft Followers

Camshaft Follower Caterpillar 3126 Diesel Engine Valvetrain Part
Venturing back inside the left side (intake side)of the block brings us to the CAT 3126 engine’s next concern: failed camshaft followers. This headache can be expensive, and while it doesn’t happen every day, lifter follower carnage can wreak some serious havoc inside the engine. And like most valvetrain failures, it can’t be resolved quickly or on the cheap. This means that, depending on the extent of the damage incurred, significant downtime, new parts, and what amounts to a costly repair are all on the table.
Causes

Cam Follower Covers Diesel Engine Side Plate Caterpillar 3126
The most common failure point here arises when the bushings that hold the rollers onto the followers fail. Unfortunately, it’s often misdiagnosed as a fuel injector issue, and many times injector swapping commences without further troubleshooting being performed. A lack of oil flow for proper lubrication, an improperly run overhead, and even incorrectly installed pushrods can lead to camshaft follower failure. As for the latter components, the pushrods have to be installed in a precise manner—and in many applications their installation can’t be viewed without the use of a flashlight and a telescopic mirror.
Solution(s)

Cat 3126 Diesel Engine Leaking Camshaft Follower Covers
More often than not, the damage incurred during camshaft follower failure calls for a new camshaft. The repair requires gaining access to both the front of the engine and the left side of it, which makes it an extensive and time-consuming repair. For most CAT mechanics, it’s a two-day affair, provided all necessary parts are obtained ahead of time. Of course, camshaft follower failure also presents the perfect opportunity to address leaking or aged cam follower covers, as well as allow for a thorough inspection of anything along the left side of the engine.
Problem #7: Fuel Injector Failure

Heui Fuel Injectors Cat 3126 Diesel Engine Parts
Containing both an oil and a fuel side, the fuel injectors used in the CAT 3126 HEUI system are fairly complex. Each one is electronically controlled via a sizeable external solenoid up top, and exotic components like an intensifier piston and poppet valves are employed internally to pull off each injection event. Essentially, each injector is its own hydraulic pump. Thanks to this complexity, it’s no wonder HEUI injectors aren’t cheap. Think $300 per remanufactured aftermarket unit, and north of $1,800 a set. Sourced direct from CAT, you’ll be looking at considerably higher prices. Reason enough to safeguard them as best you can.
What Causes It

Caterpillar 3126 Diesel Engine Heui Fuel Injector Installation
As far as premature failure is concerned, poor maintenance is the biggest cause of HEUI injector problems. Specifically, dirty oil and extended oil change intervals can take them out in short order. Running the wrong viscosity oil can also do a number on them. Internally, one of the biggest wear items is the poppet valve—the component that allows high-pressure oil into the injector so that it can act on the intensifier piston beneath it. In other scenarios, aged, brittle, and deteriorating O-rings can fail (but can also easily be rolled or cut during injector installation). Electronically, a bad solenoid can render an injector dead, too.
Solution(s)

Cat 3126 Diesel Engine Heui Fuel Injector Solenoid
Preventative maintenance goes a long way with HEUI injectors. Staying on top of your oil change intervals is paramount, as is running the correct engine oil and oil filter. To a lesser extent, low fuel supply pressure can damage the injectors as well, as can contaminated fuel. Always ensure 60-75 psi of fuel supply pressure is being fed to the HEUI injectors at all times or performance and longevity could suffer. If a failed or ailing injector is properly diagnosed, they can be replaced one at a time without the need to synchronize them with the ECM.
Problem #8: Hard Starting

Diesel Engine Cold Start Exhaust Smoke Cat 3126 Grain Truck
Hard starting and diesel engines go hand-in-hand when the weather turns cold, and it’s especially common with older, higher mile power plants or those with excessive operating hours. Weak batteries have a difficult time turning these high compression engines over, and when you combine them with an ailing or failing intake heater the hard-start issues only worsen. Despite the use of an intake heater on the CAT 3126 (there are no glow plugs), it’s an engine that is notorious for extended cranking scenarios when cold.
Why It Happens

Cat 3126 Diesel Engine Heui Oil Flow High Pressure Injection System
Aside from battery or intake heater issues, the CAT 3126’s hard-starting nature is common in most (if not all) engines with HEUI injection. In relying on engine oil to actuate the fuel injectors—and with cold, thick engine oil not exactly possessing the best flow characteristics—you’re at the mercy of the engine oil. In frigid conditions, the HEUI system will be slower to build the requisite minimum amount of injection actuation pressure (IAP) to fire off the injectors (roughly 800 psi). Hard start issues which persist on hot restarts and/or in warmer ambient temps can be caused by a faulty IAP sensor, IAPCV, or a leak somewhere within the high-pressure oil system.
Solution(s)

Cat 3126 Diesel Engine International School Bus
Unfortunately, there is no permanent fix for hard-starting HEUI engines. Without question, the 3126 is happier in warmer climates, but (of course) having a choice in operating location isn’t always possible. Our best advice here is to ensure the high-pressure oil system is in tip-top shape, make certain the batteries are always healthy for utmost cranking power, and that the intake air heater is functioning and working properly. A change in engine oil viscosity can help shorten cold starts (as can some engine oil additives), but we would only travel that road using an oil or additive recommended by Caterpillar.
Problem #9: High RPM/Overspeed (Marine Applications)

Marine 3126 Caterpillar Diesel Trawler Boat Engine
Excessive rpm failures are rare in the sense that it’s primarily reserved to marine vessels, but it happens enough that it makes our list. First and foremost, inline-six diesel engines don’t typically enjoy operating above 2,500 rpm. In fact, most commercial grade diesels are designed to operate well below that. The sea-going CAT 3126 is designed to operate in a range between 2,200 to 2,400 rpm, and in our experience, anything beyond 2,500 rpm—especially in an application where the engine is underpowered—can be borderline dangerous.
What Happens

Cat 3126 Diesel Boat Heui Engine Repair
Though Caterpillar typically sets peak engine speed at 2,800 rpm for 3126 marine applications, running it there has proven a toxic recipe for many boat owners. Prolonged periods of engine rpm hanging out near redline can result in valvetrain failures and even lead to spun bearings. This means rocker, cam follower, and cam damage, and sometimes even a windowed crankcase in the event of a connecting rod failure. Oftentimes, an improperly spec’d boat (with the wrong prop onboard) is the root cause behind overspeed failure, where the boat struggles to gain or maintain speed without elevated rpm.
Solution(s)

Boat Tachometer Cat 3126 Diesel Engine Rpm Marine Vessel Application
Just as it doesn’t tolerate overheating, the CAT 3126 doesn’t handle high rpm well. So, in addition to ensuring engine coolant temp never boils over (and stopping and addressing it immediately if it ever does), also make sure to avoid running the engine wide open for extended periods of time. In most marine vessels equipped with a 3126, the engine should be operated in the 2,200 to 2,400 rpm range. Keep heat at bay, rpm in check, and stay on top of your maintenance and the 3126 will reward you with many years of trouble-free service.
Problem #10: Injection Actuation Pressure Control Valve (IAPCV)

Cat 3126 Injection Actuation Pressure Control Valve IAPCV
As we mentioned earlier, the HEUI pump supplies the oil volume the HEUI system needs to function, while a component called an injection actuation pressure control valve (IAPCV) is in charge of creating the appropriate injection pressure. Located on the outlet side of the HEUI pump, the IAPCV controls high-pressure oil in a range of roughly 800 to 3,500 psi (800-psi for startup, 3,500-psi for wide-open throttle operation). If you’re familiar with Ford’s 7.3L Power Stroke, you know this vital part as the IPR (or injection pressure regulator). Between these two applications they are all but identical pieces.
What Happens

Cat 3126 Injection Actuation Pressure Control Valve Tinnerman Nut
Any time a 3126 has issues starting, exhibits rough idle and stalling, or experiences a loss of power with the engine under load, the IAPCV can be the culprit. Common codes associated with a bad IAPCV are 164-11, 164-2, and system fault 39. Oftentimes, it’s as simple as an aged, corroded O-ring causing the issue—and the IAPCV can be removed and re O-ringed very affordably and fairly easily (provided you have a HEUI pump with an external IAPCV). In some cases, the tinnerman nut loosens and falls off the back of the IAPCV. This rather inexpensive nut secures the electric solenoid onto the IAPCV—and the engine will die and never restart if it’s not in place. In less frequent instances, you can have an issue with the wiring harness or a damaged or corroded connector.
Solution(s)

Caterpillar 3126 IAPCV Valve Oem Heui Diesel Engine Parts
As mentioned, and provided you can access the IAPCV, its O-rings can be replaced on the cheap to get the engine up and running again. If an owner opts to start with a fresh IAPCV, they can be obtained for $300 to $350 at your local CAT dealer (PN 122-5053). If the tinnerman nut failure strikes, simply source another one and coat it with Loc-Tite during assembly (Note: do not overtighten it). However, if you have the square top HEUI pump you’ll likely have to source a whole new pump (again, the IAPCV isn’t serviceable on these). Of course, proper oil maintenance goes a long way in getting maximum service life out of the IAPCV and, indeed, all HEUI-related components. Change the engine oil every 250 operating hours, or 10,000 miles.
